  1. Submit an Application – Fill out an application which encompasses your personal details and an outline of your ongoing health condition or disability. This data enables us to tailor our support to your wellness necessities. Please also provide your physician’s name and contact details.
  2. Provide Evidence of Health Condition, Disability or Veteran Status – Furnish tangible evidence of your medical condition or disability. Acceptable proofs can be a recent doctor’s invoice, a digital scan of a medicinal prescription, or a photograph of the medication bottle label. Veterans just show official documents issued by the branch of service you served or are serving. Please ensure you redact sensitive information like prescription numbers to safeguard your privacy.
  3. Evaluation Phase – After submitting, your application will undergo an assessment. Expect a response within 7 to 10 days.
  4. Approval Notification – Once approved, you will be notified and officially inducted into MitWellness’s Compassionate Support Circle.
  5. Enjoy a Discount – As a token of our support, you’ll benefit from a 30% reduction every time you buy MitWellness products on orders amounting to $40 or above. Please note: the 30% rebate does not include any applicable taxes or shipping charges.
